The D413 suggests an authentication issue at the bank's site.
To address this:
1. Go to the Add Accounts list and select the your bank provider's login form.
2. Follow the bank's login instructions and ensure that any prompts to enter your credential are entered within 90 seconds.
3. When presented with the list of available accounts reselect all the accounts you want to connect to Money Dashboard and complete the process
Although this is going through the add process again, what in fact will happen is the existing accounts on the Dashboard will be re-authenticated rather than new duplicate instances of the accounts being imported.
